Click here to Download Aplication. Children who are six-weeks-old to kindergarten-age are eligible for enrollment in the NFCC Early Childhood Development Center (ECDC). It is your responsibility, as the parent, to provide the ECDC office with your child’s updated records.
Parents/guardians must visit the ECDC before completing an application for enrollment or being placed on the waiting list. Before your child is enrolled, a short pre-admission conference will be scheduled with the Director and potential classroom teacher. This meeting will give us (you, your child and the ECDC), the opportunity to meet and get acquainted with each other.
The Director will meet with the parents or guardians of a child with special needs at the time of enrollment. A planning session will be held with the child’s parents or guardians, a health and/or education specialist, the Director and child’s teacher(s) to plan an Individualized Educational Plan (IEP) for the child. The same committee will meet quarterly to evaluate the child’s progress.
The program, as a state licensed center, is required to maintain an updated file on all registered children. It is the responsibility of the parents to complete and return the following items: - Application – This form provides the ECDC with the necessary information about your child, as well as, where you can be reached in case of emergency. You are to notify the ECDC office in writing of any change in information for this form.
- Health Care Form – This form must be completed and signed by your child’s physician or medical provider. It must include the date of your child’s last physical exam and all immunizations.
- Emergency Card – Card must be completed and signed by the parents/guardians of each child enrolled in the program. It must include emergency phone numbers of the child’s physician, hospital preference, and a release for emergency care.
- Discipline Policy – This form states our guidance policies. You are to carefully read, sign and return this form.
- Food Service Program Form – The ECDC participates in the Child Nutrition Program, which reimburses the center for the healthy meals that are prepared for your children. In order to be eligible for this program, each parent must fill out this form.
- Tuition Agreement – This form explains the different tuition rates. You are to choose the rate that applies to your situation, sign and date form.
- Authorization and Release – This form states different areas that are required by the state for specific parental authorization (field trips, filming and photographing).
- All About Me (infants only) – This information provides a “first step” look at getting to know your child and meet the needs of your child.
- Parent Hand Book – Signed Agreement of Policies.
